Incident Report Forms
Academic Misconduct Incident Report
This form is used by faculty to report alleged academic integrity violations. If a faculty member suspects academic misconduct has occurred, they should follow the procedures listed in AP 521 - Academic Misconduct.
This form is used to ensure the safety and security of our campus community. This form allows individuals to promptly report incidents, enabling us to take appropriate actions and maintain a secure environment for all students and employees. Use this form if you have witnessed or experienced any criminal activity, safety concern, or suspicious behavior on MTC's campus or during MTC related activities.
Discrimination Report (Non-Title IX)
This form is used by any campus member (student, employee, guest) who experiences or witnesses any type of discrimination or harassment based on federally protected classes (race, ethnicity, age, gender, sexual orientation, religion, etc.) or who identifies an ADA barrier or potential barrier within the campus community (e.g., power-assist door at the library is not working).
This form is used by Marion Technical College employees and provides a confidential and structured way for employees to raise concerns, report incidents, or express grievances related to workplace matters. This input will help us address issues promptly and ensure a healthy and productive atmosphere for everyone.
This form enables individuals to promptly report injuries and accidents, allowing us to provide necessary support and implement preventive measures to maintain a safe environment for all students and employees. Use this form if you have been involved in or witnessed an accident, sustained an injury, or experienced a health-related concern on MTC's campus or during MTC related activities.
Student of Concern/Behavioral Intervention Team
This form can be submitted by any member of the campus community who is concerned about a student’s behavior, safety, or well-being. Examples include emotional disturbance, mental health crises, self-inflicted harm, suicidal intention or ideation, threats to self or others, substance abuse or misuse, aggressive behavior or violence, etc.
This form is used to report violations of the Student Code of Conduct in accordance with AP 420. The Student Conduct Officer will investigate the report to determine whether or not a violation occurred. It may also be used to document concerning or disruptive behavior even if the reporter is not sure if it is a violation or the reporter chooses to handle the situation internally to establish pattern of behavior (e.g. as an “FYI” rather than an official report or request for investigation).
This form is used by students and employees who are impacted by sexual harassment, sexual assault, domestic violence, dating violence, and stalking in accordance with 3357:10-1-45 Title IX/Sexual Harassment/Sexual Violence. When these behaviors occur, and a formal complaint is made, the Title IX office is obligated to address and remedy them and ensure that no one is denied effective access to the educational programs of the college.
Title IX – Pregnancy or Parenting Report
This form is used by students and employees to connect students who may be experiencing an academic impact due to pregnancy or parenting. For example, a student who is experiencing extreme morning sickness with their pregnancy can complete the form and the Title IX Deputy Coordinator working with pregnant and parenting students will reach out to the student to discuss possible adjustments to ensure the student is not denied effective access to the educational programs of the college.
Students can use this form to file a formal complaint with the College (excluding complaints regarding grades) in accordance with AP 440 – Student Complaints. The student complaint process is administered by the President’s Office.
Free Expression Violation or Harassment Report
Students and employees alleging a violation of their constitutional right to free expression, or harassment in relation to their protected free expression, can use this form to file a formal complaint with the College. Student complaints will be administered in accordance with AP 440 – Student Complaints and employee complaints will be administered in accordance with AP 373 – Employee Complaints. The President’s Office will review all allegations of free expression violations or harassment.
